Chronic Disease Management: The Case Manager’s Role in Preventing Rehospitalizations

This course will help healthcare professionals become familiar with various forms of Chronic Disease and provide tools to assist patients in managing their diseases at home.

About the author:
Robyn Horn has her Master’s degree in Long-Term Care Administration from the University of North Texas as well as an undergraduate degree in Psychology from Southwestern University in Georgetown, Texas. She served as a licensed nursing home administrator for 13 years which included two years at an Alzheimer’s assisted living facility. Robyn has facilitated several caregiver support groups in both facility and community settings. She is also Author of the book, “Transforming the Mundane into Sacred: Spirituality in Caregiving for the Elderly.”

Objectives:

• Identify and define the top 10 chronic diseases in the U.S.
• Explore preventative methods for reducing the risks involved with chronic conditions including exercise, performing IADLs and ADLs and diet.
• Discuss care strategies and tools to improve outcomes in care of the elderly with a chronic condition(s)
• Understand how the healthcare professional can assist in preventing rehospitalizations through patient education, empowerment and self-management

Course Outline:
1)  Learning outcomes and introduction
2)  Identify the top 10 chronic diseases in the U.S.
3)  Analyze the prevalence of rehospitalizations due to chronic diseases
4)  Explore primary, secondary and tertiary preventative methods to reduce the risks involved in developing or exacerbating chronic disease(s)
5)  Understand ways in which the healthcare professional can assist the patient in preventing rehospitalizations through self-managed care after discharge

This course is in a written and recorded format, and your certificate will be available for download immediately after successful completion of the post-test and evaluation. A score of 80% or greater is required to pass, and you may re-take the post-test if needed.
